The only way this can happen is by giving them access to an 'app' that feeds directly into your database or by having them complete these forms when with you during a consultation.

Changes to Health Provider Online Services (HPOS)

From 1 October 2015, security controls will be strengthened in line with government security standards. HPOSinfo-icon users will need to update their browsers to meet the minimum requirements for HPOS.
